Transition Process
The Right Home at the Right Time
Glow’s vision is to support Local Authorities in providing the right home at the right time to children and that they remain connected to their communities and networks. For many children, a chance of living in a long term loving foster home and family is the best outcome. We want to help Local Authorities provide the right families for younger children.
The aim of Glow’s model is to enable foster carers to build early relationships with children and young people living in our residential homes which will lead to children stepping across to live in foster care which is successful, permanent and matched robustly.
OUR AIM IS TO:
- Improve matching and finding the right homes for children at the right time.
- Improve information sharing about the child – Carers are provided with insight directly from Glow’s therapeutic and residential team, providing transparency and honesty for the child’s benefit.
- Upskill and support existing Foster Carers, allowing them to witness and spend time alongside our Therapeutic Parents utilising Glow Futures research informed model.
- Improved stability for children.
Increase permanency by ensuring a seamless transition to minimise disruption for the child or young person. - Improve outcomes for children by better care planning and transitional work, building and providing relational stability throughout and after transitions.
- Be integral to building a ‘team around the child’ model where social workers, commissioning, fostering, education and Glow residential provision collaborate and work together.
